Registering with a GP (General Practitioner).

• Find a GP Surgery (Practice) that covers the area where you live. The NHS Choices’ (online) Services directory may prove very helpful in this regard.
• Contact the GP Surgery to register with them. There, you will be asked to complete a GMS1 Form.
 
Some GP surgeries may want to see a proof of your identity and/or address. A photo identity such as a valid passport or a UK driving license is usually sufficient. For proof of address a recent utility bill may be required.
